In the interest of making health care an industry, I believe we have lost something in the quality of the medical record. The clinic where I work is always looking for ways to save money, and the Transcription Department goes under the microscope first. The docs have tried in-house, off-shore, speech recognition and a combination of all. What I find disconcerting is the resulting medical note which has everything in it from pink flamingos to anal hair patches (anal herpes). It seems that these glaring errors are not corrected in a timely or efficient manner.

I would point out that doctors make much more money being doctors than they do being medical editors which most of the despise anyway. Just getting them to speak into a machine and record the patient's visit is an ordeal. The only time the record seems to come into play is in court. There, errors like the above would not be quite so funny.

If this quest for the perfect EMR is to succeed, I would suggest that at the very least the patient and or guardian be permitted to check the EMR for veracity and to challenge anything they believe is incorrect. These corrections should then be made immediately.

I would be pretty furious if my medical record looked like some that I have seen lately.
