Q: A patient is admitted with bacteremia and the labs show E. coli infection in the patient's port. Is infection due to vascular device the primary diagnosis? 996.62?

A: I would recommend diagnosis code 996.32 bloodstream infection due to central venous catheter (port-a-cath) because this is the most specific code that applies to this condition.  However, from what is stated below, there is not enough MD documentation to support the link between the port and the infection as lab results are not a valid source of documentation on which to base diagnosis codes.  Before assigning the above-recommended complication code, there must be physician documentation specifying the link between the infection and the port.  I enjoyed answering your question and I hope that this advice assists you.

Dear Experts,

I am a researcher whose current project uses discharge-level inpatient data. There are E-codes for suicide attempt: E950 - E959. I want to know how this code is given to a patient. My concern is that there is a non-trivial chance that a suicide attempt is caused by just an accident, and without knowing the details of the incident I would say there could be mis-reporting of attempted suicides.

Who gives these E-codes for suicide attempt? Doctors or health information managers? Do health information managers update a patient' code if it turns out that the cause of inpatient stay is suicide attempt?

My apologies if my questions are not relevant for your Q&A.
