Q: What should we code for subchondral cyst of joint without mentioning osteoarthritis? Can we assign other disorders of joint 719.9x?
A. According to Dorland's Medical Dictionary, a subchondral cyst is also called a ganglionic cyst. Ganglion cysts code to 727.4x.

Q: What is the ICD-9 code for clinical depression? If you happen to have any references (i.e., coding clinic or coding guidelines) regarding this, please share.
A: According to an article on mayoclinic.com by Daniel K. Hall-Flavin, MD, clinical depression is a term used by doctors to describe major depression or major depressive disorder. He goes on to state that to meet the definition, the patient must meet the criteria spelled out in the DSM and that clinical depression is termed major depression in the DSM.
As far as something official for ICD-9-CM, I unfortunately was not able to find anything.

Q: Is there a CPT code for vasoreactivity testing with epoprostenol during a cath procedure?
A: Yes, in 2011 a new code was created for this purpose. It is add-on code 93463 and would be used with the following codes per the CPT instructions. Use 93463 in conjunction with 93451-93453, 93456-93461, 93530-93533. Report 93463 only once per catheterization procedure.
